Understanding the Scan

What is a CT PNS Scan?

A fast, non-invasive imaging test that creates detailed cross-sectional pictures of all four pairs of paranasal sinuses, the nasal cavity, and surrounding facial structures.

🔍

What PNS Stands For

PNS stands for Paranasal Sinuses — the four pairs of air-filled cavities surrounding the nasal passages: maxillary, ethmoid, frontal, and sphenoid sinuses. A CT PNS scan produces highly detailed cross-sectional images of all these sinuses, the nasal cavity, ostiomeatal complex, and surrounding bony structures — far superior to X-ray or MRI for bony detail and surgical planning.

⚙️

How CT PNS Works

The 160-Slice CT scanner rotates 360° around your head, capturing hundreds of images at different angles. Each sinus — its walls, mucosa, fluid, polyps, and drainage channels — has a different density signature. The computer reconstructs these into razor-sharp axial, coronal, and sagittal cross-sections. Coronal cuts are especially vital as they mirror the surgical approach during FESS.

🏥

CT PNS vs X-Ray vs MRI

X-ray (Waters view) only shows the maxillary sinuses and misses ethmoid and sphenoid sinuses entirely. MRI is excellent for soft tissue but poorly defines bony sinus walls and drainage pathways. CT PNS is the only investigation that shows bony anatomy, ostiomeatal complex, mucosal thickening, polyps, drainage blockages, and orbital/intracranial extensions — all in one study.

🛡️

Is Contrast Needed?

In the vast majority of cases, a plain CT PNS (NCCT) is all that is required. It perfectly evaluates sinusitis, polyps, bony anatomy, and FESS planning without any injection. Contrast CT (CECT PNS) is reserved for suspected sinus tumours, fungal sinusitis with intracranial extension, vascular lesions, or when soft-tissue characterisation is needed beyond bony anatomy.

The 4 Sinus Groups

The Paranasal Sinuses CT Evaluates

Detailed imaging of all four pairs of sinuses — plus the ostiomeatal complex critical for drainage and FESS planning.

😬

Maxillary Sinuses

The largest sinuses, located in the cheekbones on either side of the nose. Most commonly affected in sinusitis.

  • Mucosal thickening & fluid levels
  • Maxillary sinusitis (acute / chronic)
  • Dental-origin sinusitis
  • Maxillary sinus cysts & polyps
  • Ostium blockage patterns
  • Antral washout evaluation
🧩

Ethmoid Sinuses

A honeycomb of small air cells between the eyes. Critical for FESS — forms the key surgical landmark.

  • Anterior & posterior ethmoid cell opacification
  • Ethmoid polyposis
  • Ostiomeatal complex (OMC) blockage
  • Bulla ethmoidalis anatomy
  • Lamina papyracea integrity
  • Skull base proximity (Keros classification)
🧠

Frontal Sinuses

Located in the forehead above the eyes. Variable in size — sometimes absent. Frontal headache source.

  • Frontal sinusitis & mucocele
  • Frontal recess anatomy for FESS
  • Frontal sinus aplasia / hypoplasia
  • Frontal sinus osteoma
  • Posterior table integrity
  • Intracranial extension assessment
🫥

Sphenoid Sinuses

Deep posterior sinuses adjacent to the skull base, pituitary gland, and optic nerves. Rarely but seriously affected.

  • Sphenoid sinusitis (rare, serious)
  • Sphenoidal mucocele
  • Relationship to optic canal & ICA
  • Skull base involvement
  • Pituitary fossa adjacency
  • Invasive fungal sinusitis spread
🌊

Ostiomeatal Complex (OMC)

The critical drainage pathway — blockage here causes most cases of chronic sinusitis. The surgical target of FESS.

  • Uncinate process morphology
  • Middle meatal anatomy
  • Maxillary ostium patency
  • Infundibular blockage patterns
  • Concha bullosa
  • Haller cells & agger nasi cells
🔬

Nasal Cavity & Septum

The nasal passages and central dividing wall — commonly deviated and affecting sinus drainage.

  • Deviated nasal septum (DNS) grading
  • Nasal polyps — extent & origin
  • Turbinate hypertrophy
  • Nasal mass characterisation
  • Choanal atresia / stenosis
  • Septal spur & perforation
Scan Variants

Types of CT PNS Scans Available

We offer the complete range of paranasal sinus CT protocols for every clinical and surgical requirement.

Most Common 🚫💉

NCCT PNS (Plain)

The gold standard for sinusitis evaluation and pre-FESS surgical planning. No dye injection, no fasting, no preparation needed. Clearly shows mucosal thickening, fluid levels, polyps, bony anatomy, and ostiomeatal complex in all four sinus groups.

Tumour / Fungal 💉

CECT PNS (With Contrast)

Contrast-enhanced CT for characterising sinus tumours, angiofibroma, inverted papilloma, fungal sinusitis with orbital/intracranial spread, and vascular lesions. Also used when plain CT findings are equivocal.

Surgical Planning 🏥

CT PNS Pre-FESS Protocol

High-resolution coronal and axial thin-slice CT specifically formatted for ENT surgical navigation. Identifies critical anatomical variants — skull base height, lamina papyracea dehiscence, Keros classification — essential for safe sinus surgery.

Extended 👁️

CT PNS with Orbits

Combined paranasal sinus and orbital CT for cases with periorbital swelling, proptosis, orbital cellulitis secondary to sinusitis, or ethmoid sinusitis with orbital wall involvement — evaluated in a single scan.

Comprehensive 🧠

CT PNS + Brain

Combined sinus and brain CT for evaluating intracranial complications of sinusitis — meningitis, brain abscess, cavernous sinus thrombosis — or when neurological symptoms accompany sinus disease.

Follow-Up 📊

Post-FESS Follow-Up CT

Post-operative CT evaluation to assess surgical cavity healing, residual mucosal disease, synechiae (adhesions), and recurrent polyps following functional endoscopic sinus surgery — typically at 3–6 months post-op.

Why CT PNS

Benefits of CT PNS Scan

Why CT PNS is the definitive investigation for sinus disease — superior to X-ray, endoscopy alone, and MRI.

🎯

Complete Sinus Mapping

Images all four sinus groups and the ostiomeatal complex in a single study — the full picture your ENT surgeon needs before any intervention.

🏥

Essential Pre-FESS Imaging

No ENT surgeon should perform FESS without a CT PNS. It maps the surgical anatomy, identifies critical structures near the skull base and orbit, and prevents surgical complications.

🦠

Detects Fungal Sinusitis

CT PNS with its bony window settings can identify high-density fungal debris, calcification patterns, and bony erosion — vital in diabetic and immunocompromised patients.

📐

Anatomical Variant Mapping

Identifies concha bullosa, Haller cells, agger nasi cells, Onodi cells, and Keros classification — variants that significantly affect surgical approach and complication risk.

Before Your Scan

How to Prepare for CT PNS Scan

Great news — plain CT PNS requires virtually no preparation. The easiest imaging test to walk into. Follow these simple guidelines.

Do's – Before Your Scan

  • Arrive 15 minutes early for registration — very little else is needed
  • Bring your ENT doctor's prescription and any previous X-rays or CT reports
  • Inform staff about all medications — especially nasal steroids, anticoagulants
  • Tell us if you are pregnant, breastfeeding, or have a known contrast allergy
  • Continue your nasal sprays and antihistamines as usual before the scan
  • Fast 4–6 hours only if CECT PNS (contrast) has been prescribed
  • Mention any history of diabetes — relevant for fungal sinusitis risk assessment
  • Stay as still as possible during the scan — very important for sharp images
🚫

Don'ts – What to Avoid

  • Do NOT eat or drink 4–6 hours before contrast CT PNS (if prescribed)
  • Avoid wearing earrings, nose pins, spectacles, or hair accessories
  • Do not carry keys, coins, or metal objects into the scan room
  • Avoid nose blowing forcefully just before the scan
  • Do NOT use nasal decongestant sprays immediately before scan (may mask findings)
  • Avoid swallowing or talking during the brief scan period — stay very still
  • Do NOT take Metformin 48 hours before and after contrast CT (consult doctor)
  • Avoid delaying — chronic sinusitis complications can progress to orbital or brain involvement
Step by Step

What Happens During
Your CT PNS Scan?

One of the quickest and easiest CT scans available — most patients are done in under 15 minutes total.

1

Check-In & Registration

Present your ENT prescription at reception. Staff verifies your details, prepares your file, and records any allergies, medications, and relevant history — takes just 3–5 minutes.

2

Remove Metal & Prepare

Remove spectacles, earrings, nose pins, and any metal accessories. For plain CT PNS, no gown change is needed. You simply lie down comfortably on the CT table.

3

Positioning

You lie on your back on the padded CT table, head in a padded headrest. Your head is positioned in the gantry for both axial and coronal sinus images. The table slides smoothly and quietly.

4

The Scan (5–10 min)

The gantry rotates silently. Each CT pass takes just a few seconds. You will be asked to keep very still — no breath-holding is usually required for CT PNS, unlike chest scans. The whole scan is complete in minutes.

5

ENT-Format Report Same Day

Our MD Radiologist prepares a structured, ENT-focused report with Lund-Mackay scoring, anatomical variants, and findings across all sinus groups. Delivered same day via WhatsApp and email for your surgeon.

📋

What the Lund-Mackay Score Means

Our radiologists provide the standardised Lund-Mackay CT Score — the internationally accepted system for grading sinus disease severity. Each sinus group is scored 0 (clear) to 2 (completely opacified), and the ostiomeatal complex 0 or 2. Total score out of 24 guides your ENT surgeon's decision between medical management and surgery. Most ENT surgeons in Ahmedabad require this score before approving FESS.

⚠️

Sinus Complications Need Urgent CT

Complications of acute sinusitis — orbital cellulitis, subperiosteal abscess, meningitis, or cavernous sinus thrombosis — are medical emergencies. If you have eye swelling, vision changes, severe headache with fever, or neck stiffness alongside sinus symptoms, please walk in immediately. Our CT scanners are available 24 hours for emergency imaging.

Common Questions

Frequently Asked Questions

Everything you need to know about CT PNS Scan in Ahmedabad — answered clearly.

CT PNS (Paranasal Sinus) scan is the gold standard imaging investigation for the sinuses and nasal cavity. It is done to evaluate chronic sinusitis, nasal polyps, sinus tumours, deviated nasal septum, facial pain, and sinusitis complications. It is also the mandatory pre-operative scan before FESS (Functional Endoscopic Sinus Surgery). CT PNS is far superior to X-ray — it visualises all four sinus groups, the ostiomeatal complex, and critical surgical landmarks that X-ray and endoscopy cannot show.
At Usmanpura Imaging Centre, NCCT PNS (plain CT without contrast) starts from just ₹2,000 — significantly more affordable than hospitals and private centres in Ahmedabad where the same scan costs ₹3,500–₹5,000. CECT PNS (with contrast) starts from ₹3,500. All prices are transparent with no hidden charges. Insurance is accepted. Call or WhatsApp us for the exact current rate for your prescription.
For plain CT PNS (NCCT), there is virtually no preparation required — it is one of the easiest scans to walk in for. You can eat, drink, and take all your normal medications. Simply arrive 15 minutes early, remove metal accessories, and you are ready. For CECT PNS (with contrast), fast for 4–6 hours before the scan. Always inform our staff about any contrast allergy or kidney disease before contrast is used.
CT PNS is one of the fastest CT scans — the actual scan takes only 3–5 minutes, and total time at the centre including registration and preparation is usually just 15–20 minutes. No breath-holding is required unlike chest scans. Your comprehensive radiologist report with Lund-Mackay scoring is ready the same day — usually within 2–4 hours. Delivered via WhatsApp and email. Hard copies and CDs available at the centre.
Yes — absolutely mandatory. No responsible ENT surgeon should perform FESS without a current CT PNS. The CT maps all critical surgical landmarks: skull base height (Keros classification), lamina papyracea integrity, ostiomeatal complex anatomy, position of the optic nerve and internal carotid artery relative to the sphenoid sinus, and all anatomical variants. This information is essential to perform FESS safely and effectively. Most ENT surgeons will specifically request a CT PNS report with Lund-Mackay scoring.
Yes — CT PNS is excellent for detecting nasal polyps, mapping their origin, and assessing their full extent throughout the nasal cavity and sinuses. It shows whether polyps are bilateral, which sinus groups are involved, how much of the nasal cavity is obstructed, and whether there is associated bony remodelling or erosion. This information guides medical management and surgical planning. CT PNS is superior to nasal endoscopy for mapping posterior sinus polyp extension.
CT PNS uses a low dose of ionizing radiation — significantly less than a CT of the abdomen or chest. Our 160-slice CT uses advanced low-dose protocols to minimise exposure while maintaining excellent diagnostic image quality. The small radiation dose is well within internationally accepted safety limits and is far outweighed by the diagnostic benefit. CT PNS is generally avoided during pregnancy unless absolutely necessary — always inform our staff if you are pregnant.
Yes — CT PNS is particularly important for diagnosing fungal sinusitis, which is more common in diabetics and immunocompromised patients. CT findings of high-density (hyperdense) material within the sinuses, scattered calcifications, and bony erosion are characteristic of fungal sinusitis. CECT PNS can further assess soft-tissue involvement and any orbital or intracranial spread. Fungal sinusitis is a surgical emergency — urgent CT PNS is needed for immediate diagnosis and treatment planning.
