What is an Abdomen X-Ray Scan?

An Abdomen X-Ray (plain abdominal radiograph) is a simple, rapid imaging examination that uses a carefully calibrated beam of X-rays to produce a two-dimensional image of the abdominal organs and structures. The resulting image, captured on a digital detector panel, shows various structures based on their density – gas (black), soft tissue (grey), fat (darker grey), and bone/calcium/metal (bright white).

The most common views include the Erect AP (upright, front-to-back) – which is the primary view for detecting free intraperitoneal gas (perforation) and air-fluid levels (obstruction); the Supine AP – used to assess bowel gas distribution, organ sizes, and calcifications; and the Lateral Decubitus – used when the patient cannot stand, to detect small amounts of free air.

A KUB X-Ray (Kidneys, Ureters, Bladder) is a specific abdominal X-ray focused on the urinary tract for detecting radio-opaque kidney stones, ureteric calculi, and bladder stones. It covers from the upper poles of the kidneys to the bottom of the bladder in a single image.

Free Gas (Perforation)

Air under the diaphragm on erect X-ray is the classic sign of bowel or visceral perforation (perforated peptic ulcer, bowel perforation) – a surgical emergency. Erect X-ray is the first test ordered.

Bowel Obstruction

Dilated loops of small or large bowel with multiple air-fluid levels on erect X-ray indicate mechanical bowel obstruction. Distribution of dilated bowel helps distinguish small from large bowel obstruction.

The most clinically important abdominal X-ray view. The patient stands upright for this projection. Essential for detecting free subdiaphragmatic gas (bowel perforation) and demonstrating air-fluid levels in bowel obstruction. Always the first view requested in acute abdomen.

The patient lies on their side – a useful alternative to the erect view for detecting free intraperitoneal air in patients who cannot stand. The left lateral decubitus position (right side up) is preferred for best free gas detection.

How to Prepare for Abdomen X-Ray

Abdomen X-Ray is one of the simplest imaging procedures. Minimal preparation is required in most cases.

  • For a routine abdomen X-ray, no special preparation is usually required – walk in without fasting
  • For KUB X-Ray, drink 2–3 glasses of water 1 hour before the scan to fill the bladder for better visualization
  • Wear loose, comfortable clothing or a hospital gown will be provided
  • Bring your doctor's referral slip and any previous X-ray or scan reports for comparison
  • Inform our staff if you are pregnant or think you might be pregnant before any X-ray
  • For erect (standing) X-ray, inform our team if you are unable to stand due to weakness or pain
  • For bowel preparation (sometimes requested by your doctor before contrast studies), follow their specific instructions
  • Continue all regular medications normally before an abdominal X-ray

❌ Don'ts

  • Do not wear clothes with metal zippers, buttons, belts, or hooks – these cause artefact on the abdominal image
  • Do not keep jewelry, keys, phone, or metallic objects in your clothing during the X-ray
  • Do not conceal pregnancy from our staff – abdominal X-ray is avoided in first trimester unless a medical emergency
  • Do not move during image acquisition – even slight movement causes image blur
  • Do not eat a heavy meal immediately before a KUB X-Ray – bowel gas from food can obscure small stones
  • Do not apply abdominal ointments, pastes, or metallic medicated patches near the area being X-rayed
  • Do not delay an emergency abdominal X-ray – if you have acute severe abdominal pain or vomiting, come immediately

Plain abdominal X-ray (KUB) can detect approximately 80% of urinary tract stones – specifically radio-opaque stones containing calcium (calcium oxalate, calcium phosphate). However, pure uric acid stones are radiolucent (not visible on X-ray) and account for about 5–10% of stones. Cystine stones are faintly radio-opaque. For complete stone detection and characterization – including small stones, radiolucent stones, and precise location – Non-Contrast CT KUB (NCCT KUB) has near 100% sensitivity and is the gold standard. Your doctor will advise which investigation is appropriate for your clinical situation.
The combination of erect and supine abdominal X-rays (the "acute abdomen series") provides maximum information in acute abdominal presentations. The erect view is essential for detecting free subdiaphragmatic gas (sign of perforation) and demonstrating air-fluid levels in obstructed bowel loops (the hallmark of mechanical obstruction). The supine view shows bowel calibre, overall gas distribution pattern, and calcifications. Together, they provide complementary information that neither view alone can provide, which is why most emergency departments request both views simultaneously for acute abdominal presentations.
Abdominal X-ray during pregnancy requires careful consideration. In the first trimester (first 12 weeks), it is generally avoided unless there is a genuine medical emergency where the benefit outweighs the risk, as the developing fetus is most radiosensitive during this period. In the second and third trimesters, the risk is lower, and an abdominal X-ray may be performed when clinically essential.
